Printed on two sides, 11 x 8.5", fold, "OK" and checkmark at top (and other notes) in pencil (in another hand) else very nice, stapled to several other sheets of photocopied biographical and bibliographical information. Prepared for inclusion in a reference book on authors. Frazetta lists his parents' and wife's names, educational background, home address (PO box), Ralph Bakshi; he signs and dates the questionnaire at bottom of verso. Neat.
