One page, 8 x 5", printed and handwritten, paper toned, some staining at edges, folds, edge tears, creases, but still an interesting little survivor. On July 18, 1924 a doctor in Toledo, Ohio gave a primary inoculation of the bovine virus to a seven year old male who lived at 4142 Vermaas in Toledo. The doctor dated and signed at the bottom on July 15, 1924.
