8 x 4.5", leather-like cloth book (with doctor's name printed in gilt on front cover) containing six mounted cards for testing eyes: one printed text (60 to 20), one with silhouette symbols and numbers, one showing playing cards, one page with excerpts from phone book, stock prices, a train schedule and shorthand, and one with musical notation; the book is soiled and worn, extremities fraying, outer spine splitting, contents are toned and soiled, but still an interesting piece. Gilt name on front cover: Charles Porter Small, MD was the first physician in charge of student health at the new University of Chicago. In 1897, after studying ophthalmology in Vienna he returned to Chicago to practice his specialty which he did for forty years.
